- Must show all the products you want listed on the certificate.
- The buyer must be listed with a US address.
Good Manufacturing Practices Statement

The statement must:
- Must contain all the products you want listed on your certificate.
- Must be on a company letterhead, dated, and signed.
- If purchasing from the wholesaler, we suggest that you compose the manufacturing statement yourself. All you need to do is name the company that manufactures the products you want listed on your certificate.
Good Manufacturing Practices Statement
- A statement written on letterhead from your company attesting to your company's adherence to all relevant Good Manufacturing Practices.
